Preparing For Your Visit

If it's your first visit with us, please plan to arrive 15 minutes before your actual appointment time to complete the registration forms. Completing the forms in advance of your appointment will facilitate the processing time required for your first appointment and allow you and your provider to optimize your time together. If you've completed the necessary forms in advance, please arrive at least a few minutes early.

Parking
There is street parking available all around the building. There is also a pay lot directly across the street; parking fees are $2.00 for up to 2 hours.

We strongly encourage you to contact your insurance company prior to your appointment to confirm that the provider you have chosen is a provider on your plan and that you understand what your benefits are.
